If the employer did not include its contribution to your HSA in box 12 with code W, then, yes, you can add that amount to the dialogue that you referenced: “Did your employer tell you about any other contributions? Employer and payroll contributions not reported in Box 12 of your W–2.”
Why are my HSA contributions not on W2?
The portion of the HSA that was paid by the employer should be in Box 12, code W. If your employer made a payment to your HSA and did not report it on your W2, they need to issue a corrected W2. You should not file your taxes until you receive it. If the payment was made by you, it will not appear on the W2.
